When you go into your movie libary in apple tv and select into a film there is a list of actor at the bottom with names and a profile picture icon. How can i add a picture to the actor? Or why isnt it show up and how can i fix it?

Unfortunately this is not possible at this time. You are welcome to share your feedback with Apple, using the following link.
Apple has their own collection of cast & crew images, and looks it up when such information is accessed. That collection may not be complete, or the image server may time out for a response. The photos are not part of the meta data of the media files themselves.
(It even works for Home Sharing content sometimes, if cast & crew names are added, but it is hit-and-miss, even celebrities may not load.)
What might the reason be that some show and others dont?
