How do I prevent Personal Hotspot from automatically activating whenever a device decides to connect to my phone

Your devices are using Instant Hotspot, a Continuity feature.

It is triggered by using the same Apple ID and Bluetooth.

It is one of many reasons for not sharing an Apple ID. If you insist on disregarding Apple’s recommendations there will be consequences, and this is only one of them. See the 4th answer here—>https://appleid.apple.com/faq/#!&page=faq

It’s fine to share an Apple ID among devices used by one person, but not by devices used by multiple individuals.

On the iPad connect to the hotspot, then tap the i next to the network name and Forget This Network. I don’t know if this will work, but it does for me.
