do I have to pay $4.99/ month per box if I have multiple boxes on one house ?

The Apple TV+ subscription is for an Apple ID account, not for a specific device. If you use the same account on multiple boxes (or use Family Sharing), then that as fine with just one subscription.

There is a limit on simultaneous streams per account per service, and for Apple TV+ that is 6 simultaneous streams. You can have it on more devices, but not use more at the same time.
