iOS 13.5.1 Rear Camera Issue

My iPhone X's rear cam is blurry when taking photos of objects over 2 meters away from me.

It is also consistently blurred when the camera is tilted or facing down below the normal horizon level(just like in when taking shots of notes in my notebook).

There is also a low ground sound when the unit is video recording.

The unit is currently in iOS 13.5.1

What causes this problem? Is it a hardware issue or a software issue? I've been trying possible solutions online but none helped.
