i bought Apple Watch in UAE
I am unable get the ECG

ECG is viewed as medical equipment and is illegal to sell in places without the approval of the government
watchOS - Feature Availability - Apple
ECG will not work until both the country of purchase and the country you reside in has approved it's usage.
