It is a developer tool for testing.

Are you a registered developer?

Info:

TestFlight makes it easy to invite users to test your apps and app clip experiences and collect valuable feedback before releasing your apps on the App Store. You can invite up to 10,000 testers using just their email address or by sharing a public link.

You say that this is your 3rd iPhone in 2 months. Where are your previous two iPhones? Did you remove your Apple ID from them and perform a factory reset prior to delivering them to their next owners? If not, then these phones are still registered as trusted devices on your Apple ID account and can make changes to the account without asking permission from you (because you never deprived them of that permission).

Click on this link (What to do before you sell, give away, or trade in your iPhone, iPad, or iPod touch - Apple Support) and follow the directions for how to remotely erase a lost device and how to remove it from your Apple ID. Good luck.

An iPhone can't be "hacked". There also aren't any "viruses" for iPhone. If you are encountering issues of compromise, its related to the accounts that are associated to your device, not the device itself. Ensure you are using strong passwords as well as any available additional security measures, such as 2 Step Verification or 2 Factor Authentication. This will ensure your accounts are harder to breach or compromise. See: https://support.apple.com/en-us/HT204915 for Apple's 2 Factor Authentication.

It has already been described. It can also be used to install apps that did not pass muster to be in the app store. Such apps can contain spyware, so you should never install an app that did not come from the app store.

If you have it on your phone you should remove it, unless you are testing an app for someone you know and trust. To remove it:

Go to Settings/General and scroll to the bottom. Tap on “Profiles” (just below VPN), tap on the Profile and remove it. Then restart your phone.
