If you have iOS 13.4 or later follow:

If you forgot your Screen Time passcode - Apple Support

If not you must erase your device the same way as if you forgot passcode, and not set it up with a backup.

If you forgot the passcode on your iPhone, or your iPhone is disabled - Apple Support
