Ask your provider for a replacement on the same number.
Were you not asked to remove it before sending off?
He said "sent back". I assumed he did not use a store.
It is polite to add to responses from other members, but not to repeat parts, particularly after two hours.

Hi, it would be best to go back to the store where your phone was repaired & ask to speak with the manager as you may still be able to retrieve your SIM card back. Otherwise, you can request a new SIM card from your carrier with the same number.
Indeed, a visit to a store often takes the form of a phone call in today's world. Furthermore, my last two service interactions with Apple began with a personal visit to the store and ended with the repaired/replaced device being shipped directly to my home. Had I had a problem either time, I would visit the store in person. I would, and could, do so today.
Tried that but it did not help. I purchased my phone from Walmart with a straight talk plan. The Walmart clerk said he could only give me a sim for a new purchase or a phone being brought over from another provider. Called Straight talk and ordered a new sim for 99 cents (plus tax) with free two day shipping. Apple gave good service. phone was repaired day after received and shipped back same day as repaired.
