Duplicate contacts in Icloud and Iphone

I have three of every contact showing up in my iphone contacts and Icloud contacts. I sync those with me Outlook contacts, which is only showing one contact card for each person as it should. How do I remove the duplicate contacts on my iphone and icloud contact lists?
