Had the same issue on the public release of IOS 14. Reset (network settings, complete factory reset), removing VPN, removing sim card etc did not help. I'm with Visible on Verizon Network with an iPhone SE 2020 (2nd gen).
Only solution was to connect the iPhone to Itunes (on Windows 10 in my case). Once I connected a message "carrier settings updated" appeared on my iTunes on Windows. And then the hotspot setting was available again and is working now! Very odd

Thank you Fritz - I did all the same things you did and what ended up working for me is changing the mobile data network APN - my carrier is defaulting to a data network which does not support personal hotspot. So when I did a network reset it defaulted to that one every time.
My carrier even told me the wrong settings for the APN and were unable to help. Told me I had to send my phone off for repairs. I then Googled and found APN settings for my carrier. Manually entered those in and powered my phone off and on. Personal hotspot is back. So for those in Australia your APN settings for Telstra need to be Telstra.iph not Telstra.internet. Have a great day.
