When on FaceTime on my ipad mini 2 (ios 12.4.8) it returns me to the home screen with the green 'return to call' bar at the top and I unable to return to the FaceTime when tapping it and it doesn’t appear in the app switcher. It happens nearly every time I use ft making pretty much unusable

Go to Settings > General > Date & Time and turn on Set Automatically if that’s not enabled and then force close the FaceTime app:

Restart your iPad after that:
If that doesn’t help, delete the FaceTime app, restart, and reinstall the app and see if this has any impact on this.
