ECG and OXYGEN apps on watch

my new Apple Watch 6 was sent to me by a US friend few days back through a passenger.

I am a medical doctor with spinal cervical partial compression.this is non operable here with a better outcome

And I get arrhythmia mostly due to same reason.

This watch was most wanted gift that I could receive.

But unfortunately my both apps are not working here.Can you advice how to get it rectified?
