iPhone Disabled

My 92 year old mother's iPhone 6S has a message saying iPhone is Disabled - connect to iTunes. I connected to iTunes on my computer to try and restore her phone to reset the passcode. I ended up with a message saying that the phone could not be found. How can I reset her phone so that it is usable for her?
