Do I need VPN on my iPhone?
I use my phone (when on internet) as a hotspot at home (Verizon).
I do not access the internet via any outside wifi connection.
I live in a rural area and no one can connect to my phone’s wifi network.

No. You don't need it regardless of how you connect.
