How do I create a @Mac.com email?

@mac.com addresses can not now be created - they haven't been for some years. Only @icloud.com addresses can be created, either when opening a new iCloud account or as 'email aliases' which are additional addresses within an account. (Existing @mac.com addresses, held by people who were members of the old 'Dotmac', continue to function.)
