Bluetooth to iPhone with Roger Pen (Phonak)

I recently acquired a Roger Pen which is a device designed to help the hearing impaired. It transmits audio from an embedded microphone or from Bluetooth or audio cable connected devices and relays it over very low power FM directly to compatible hearing aids.
I have successfully paired this device with an iPhone 8 Plus and can accept telephone calls or even place voice dialed calls in a manner similar to hands-free operation in automobiles. This all works as expected.
When the Pen is connected to the iPhone via an audio cable, the Pen is recognized as a headset, the iPhone speakers are disabled and all audio output is transmitted via the Pen to the HA. This is as expected.
However, when connected via Bluetooth what doesn't seem to work as expected is viewing YouTube videos, playing music, listening to iBooks and probably many other apps with audio output. In these cases, the audio is played on the iPhone speakers and not transmitted over Bluetooth to the Roger Pen and thence to the HA. Instead, the Pen microphone picks up the audio from the iPhone speakers and transmits that to the HA.
I may be missing something, but have been unable to get this last scenario to work. Any ideas?
