What if the “camera” button does not appear...
What if the “camera” button does not appear at the bottom of the new or existing note...the only options I see are delete, create new (pencil) or upload....

Welcome to the Apple Support Communities. It sounds like there are issues with the 'camera' option not appearing on the note. We'll do what we can to help.
If you are using a different mail account for notes, they might not allow photos as an option. If possible, try starting a new note under iCloud or on the iPhone itself. Also, check the default account for the Notes app (Settings > Notes > Default Account): Use Notes on your iPhone, iPad, and iPod touch
