Camera Recently Used By “Unknown”

Continuously seeing the green dot turn on notifying that my camera is being used then when I swipe down and it states

“Unknown, recently “

This happens every day, then my service signal goes from full bars to one or two bars when I haven’t even moved the phone. Very strange and seems to just get worse to the point of my phone not sending or receiving anything.
