Are you using the latest iTunes version on your PC or your Mac?

Have you performed the specific gestures provided at points #1 & #2 at section "Remove the passcode on an iPad that has a Home button" in this support article?:--> If you forgot the passcode on your iPad, or your iPad is disabled - Apple Support
