Do I need to remove the screen protector before returning an iPhone?

While I don't believe its a written requirement, I remove screen protectors and the sim card before I turn in any phone.

If you are mailing the phone back, make sure you take photographs of the phone from all angles in case there is any dispute about the device once received.
