My phone icon disappeared from the bottom line of my phone. How do I get it back?

Press and hold the Phone icon. Drag it onto the bottom line (dock).

Open Settings ➔ General ➔ Reset ➔ Reset Home Screen Layout : This should return your home screen icons as they were.
