Document won’t rename from title bar

It looks like this problem goes back to at least 2016! Has anyone found the problem?

In my case this is a system-wide issue which doesn’t happen if I choose any new location other than where the file was opened/created OR I create a new user account. Is there some file/setting that is the problem?

I’m hoping someone has found what the difference is between the “clean” user account and the normal user account.

When you open a file you see the title at the top of the window,

move the cursor over the title at the top of the window

and you will see a downward pointing arrow which when clicked

on will allow you to change the files title. The destination of the file

can be altered to by selecting one of the predetermined folder/ locations

from the dropdown menu next to Where:, or if you click on Other…

you will get the full folder navigation pane.
