No, do not send anything but the iPhone, it should tell you this is the material you received. The only Apple trade-ins where the power adapter might make a difference is for Macs.
Just before you put the iPhone in the box take a short video of the iPhone working and take copious pictures of the iPhone itself to document there is no damage.
