How do I get the QR code from my phone to the TV for my subscription

Exactly as indicated:

To scan, use your camera app or a QR code reader on your device.

• Use the iPhone Camera app to bring the code into view. It will recognize it as QR code, and offer in a pop-up notification to use Safari to open the URL.
• or: Use the Code Scanner widget in Control Center. It will scan the QR code, and when it finds that it represents an URL, then it will go to that website.
• or: Use any QR code reader app to do the same.

It is nothing special. It is just a shortcut to https://activate.apple.com. You still have to manually enter the code.

The QR code is the one you see on your TV. That is the one you need to scan.
