Cards deleted from Wallet

Credit cards, including Apple Card, have been deleted from Wallet, and I can not add them back in.

If you have actually signed out of iCloud on your iPhone, then I would wipe out and reset the phone.

Then log back into the iCloud account - that should restore the Apple Card.

As for your other cards, you will have to manually add them back on (requires card authentication).
