In Settings > General > Software Update > Customize Automatic Updates, do you have both the 'Download iOS Updates' and 'Install iOS Updates' options enabled? Also be sure to note that the page does mention "Some updates might need to be installed manually." The automatic updates will happen overnight, and while the device is locked, connected to power, and connected to Wi-Fi. If you're not regularly having it connected to Wi-Fi and power overnight, that may be playing a part, as well.

Thanks for clarifying the issue. We note you mention having the devices connected to Wi-Fi and plugged in. Are both of those applicable overnight as well? We ask as that'll be when the automatic updates would take place. How much storage is available on each device? If both are very low on available storage, then this could be a cause. Try also connecting to a different Wi-Fi network on the affected devices, if possible, to help rule out any configuration issues with the existing network.
Is this happening for a long period of time after an iOS update releases before attempting the manual install? For example, does it still not install the updates after a couple weeks?

I have everything set correctly (as stated in the original post).
BTW - "Some updates might need to be installed manually." doesn't appear.
I have NEVER had the updates download and install automatically, despite the device plugged in, and connected to WiFi. I guess the only thing I can't control is NBN - if it drops out at an inopportune time, then the update probably won't happen. But then why doesn't the iPhone / iPad keep trying? (did I mention I have the same problem on the iPad?)

Yes, overnight (as per instructions / advice from the devices).
iPhone has 11G (of 32G) available, iPad has 12G (of 32G) available - more than enough, I would have thought.
I only have 1 WiFi network to work from (all my neighbours have secure WiFi networks!), although I have a choice of 2 frequencies. Everything else seems to work ok on the same WiFi network.
I usually attempt the update within a few days of the advice - it prevents getting repeated reminders that I have updates to install.
I haven't tried manually downloading, then auto install, although if I'm manually downloading, then I might as well manually install.
I attempted to contact Apple support online, but it basically said I was out of the support window, at least for the iPad, and the iPhone is a refurbished unit, although my previous unit (iPhone5) had the same problem.
If it makes any difference, the modem is a Foxtel unit.
