I had to choose “Allowed Websites Only” in Screen Time-Content Restrictions-Web Content and then toggle back to “Limit Adult Websites” and it works again.

On a Mac, use the Safari File menu New Window instead of New Private Window.

But how to disable it? Not just turn it off/on. Content restrictions are still on, but now private window is an option. Don’t want this.
