Can a name be changed in a group chat without intentionally changing it

Can a name be changed in a group chat without intentionally changing it. Had a situation with 20 kids on a group chat and it seems when someone one go into the group chat and text it would change the group chat name to a previous one. Unfortunately it has caused a lot of drama as the name changes were not warranted
