iPhone Trade In Kit

I had a new iPhone delivered on Sunday, and I need to send in my old iPhone. I haven’t received a Trade In kit. How do I request one? Thanks!

What is the trade in kit? All i have received is my phone and the box it came in? how do i complete a trade in when i purchased the iphone online? i haven’t received a trade in kit yet when i thought it would come along with the new phone?
