You will need to get that from the company that provides your phone service. Apple does not handle this.
Use a SIM PIN [PUKcode] for your iPhone or iPad --> https://support.apple.com/HT201529
Ask your carrier to help you unlock your SIM card using the default SIM PIN or PUK code.
