Monitor elderly via phone or watch

I have an iphone I would like to remotely monitor my elderly parent that is also using an iphone and iwatch 6

1) real time GPS location

2) health data from watch and health history

Can this be done natively via iphone or icloud?

If not, is there an app that can do that?

Not really and no. Check with the parents doctor regarding some other means to do this.
