Unknown devices on Apple TV
We were watching tv and a strangers device popped on the screen with a four digit code “Lisa’s iPad 1234”. Does this mean they were able to access our Apple TV or just attempting to?

They were attempting to connect. To actually connect, they would have needed to see the four digit code, and type it in to their device.
Check your AirPlay access settings. Set it more strict to prevent others from connecting.
