Are you perhaps leaving your iPad connected to its Power Adapter for extended periods? If so, this is not an issue - but this can alter the iPad battery charging strategy.

Your iPad will suffer no harm in being connected to external power for extended periods - whether in use or not. This usage model is common in Point of Sale (POS) and Kiosk applications. The iPad will optimally charge your iPad battery, modifying its charging strategy, as appropriate, to reduce battery chemical aging.

This could be related to your software and the only good way to find out is if you do a factory reset and put a clean install of iOS back on your iPad.

Follow these articles and it will get you back up and running. Please make sure you have a recent backup or go ahead and make one.
