More than likely that app is still on the iPhone and is in the App Library. The App Library is the last screen on your iPhone when you continue to SWAP LEFT until you reach it. In the App Library, find that app that you are looking for and then TOUCH & DRAG it out of there and on to your favorite Home Screen.
