How can I use it and Is it work for 5th generation?

Getting this from Costco:

It's sold by a third party, and Apple would be the one who provides to coverage (through AppleCare). But, on a personal note, and based on prior experiences, I'd stick with the manufacturer, and just purchase it from Apple, for higher assurance of coverage.
