One time I set the alarm on my phone at 4 am and took it off, now every morning at 4 am the alarm comes on by itself how you fix it.
My alarm on my phone comes on without being set at 4 am very annoying how you fix it.

Is this alarm appearing in Bedtime? If so, turn Bedtime off and see if this resolves the issue. You can read more about this here:
