I am still being charged Apple Care for a phone I returned

I was charged Apple Care, which I did not want. I returned the phone and was informed the Apple Care would be removed as well. I was recently charged for Apple Care for a phone I returned within the allotted time frame. How do I remove this subscription from my account?
