How do I remove these duplicate contact listings and equally important, how do I prevent them from happening in the future?

Please see the following article on how to remove duplicate contacts on your Mac: If you see duplicate contacts after setting up iCloud Contacts - Apple Support

There are no foolproof ways of preventing this in the future. Duplicates contacts can emerge from:

  1. iCloud synchronization
  2. Contacts from other email accounts

in case #1, your only step is to remove the duplicates as shown in the above link. In case #2, you may choose to merge those duplicate contacts into one single contact if they share the same name; This functionality should be on by default, though.
