My son downloaded a game that had no price to it but it charged my debit card $99.99. Why is this and how can I get my refund back?

You can request a refund by this procedure:
Request a refund for apps or content that you bought from Apple - Apple Support
You can communicate with the credit card company to cancel the deduction because the software is suspected of misleading consumption. 😄
This free game probably contains "in app purchases", or "in game purchases". I don't know if you can get your money back.
