Screentime notified me that I was using my iPhone for nearly 24 hours a day. The top used app is this mystery “dietitianguide.club” running constantly. I set a 1 minute time limit in screen time and ran a bitdefender scan, but I still can’t find the mystery app anywhere. Any idea what could be going on?

That's not an App, it's a website. Open Safari and see if you have a tab running with this dietitionguide.com. If you do, close the tab.
You can also go to Settings > Safari > Clear History and Website Data, which will close the site down.
