Contact your carrier and ask them if everything is okay on their side.

Should be this case, follow all the steps provided in this support page:--> If Personal Hotspot is not working on your iPhone or iPad (Wi-Fi + Cellular) - Apple Support

If the issue persists, contact Apple Support directly, using this link:--> https://getsupport.apple.com/?caller=kbase&PRKEYS=PF9&category_id=SC0068&symptom_id=23245
