My wife's Apple watch doesn't display an intercom icon, and when you use the Siri intercom functionality it displays a message "to do that turn on intercom setting on home app setting" when the setting is enabled everywhere I can think of.
The icon is available on the IPhone the watch is paired with and the intercom function works fine using the icon and Siri.
iPhone and watch software is up to date.

The watch and iPhone are using versions of their operating systems above the minimum requirements.
But the intercom option is missing from the watch and intercom is enabled on the Home app on the phone (there's no separate option I can see on the watch).
When you use Siri on the watch with the keyword Intercom, the watch lets you record a message and then says that to broadcast it you need to enable the intercom feature in the home app.
