iPhone 12 Mini voice/music volume issue

Ok, here's a tough one:

When I enable the volume buttons to adjust the ringer and alerts volume in -Settings-, occasionally I'll open an app or unlock my phone and press the volume buttons on the side and it will adjust the volume for an app on the left side of the screen. Even though all my apps are closed out. Not the ringer volume on the top of the screen. I will lock my phone an no sound happens (the lock screen sound). I will have to go into Settings to adjust the ringer volume even though I should be able to adjust it on the side with the volume buttons.

When that happens and I'm driving with CarPlay, it will play Pandora and Spotify with "Voice Volume" and not "Audio Volume". The sound is slightly distorted. I have to restart my phone every time that happens. This has been going on for a long time. Any solutions? I've switched off the "Change with Buttons" feature to see if that will help with CarPlay Audio.

Thank you!
