I have scanned documents in the past, so it was there once. iphone 6s. Have most recent iOS version. Suggestions--besides getting a new phone? :-)

Do any other options appear which would normally be next to the camera symbol, such as table or font?
No, they weren't, but I was able to discover a solution: Notes was not turned on in my iCloud settings. Once I did this the toolbar (with camera icon and others) appeared.
