I have the imei number how can I fill out the form to have the phone reset completely?

IMEI isn’t a key that allows resetting Activation Lock. Nor does the serial number. (That’d be a catastrophe.)

You’ll need a purchase receipt showing the serial number or IMEI, and a discussion directly with Apple, to reset Activation Lock.
