how do you adjust the volume in the TV remote app?

I can adjust volume using the standard volume buttons. Not all TVs support that.

The volume buttons on the side of iPhone and iPad may be used with AV receivers that support this feature when HDMI-CEC is activated. TVs generally do not support volume control through HDMI-CEC.
