Don't try to sync media via Windows Explorer, except for tones/ringtones.
Try Edit > Preferences > Devices > Reset Sync History (if available) and then try syncing once more.
If that hasn't helped try signing out of the iTunes Store on the device and then try again.
Otherwise see this post on erratic syncing.

I actually worked on this about a week ago for HOURS! Tried every reset... Deleted the whole library... etc etc etc. I downloaded CopyTrans Manager. I have only done 1 MP3 so far... but it worked! Now... to try with the rest of my 2,000 songs I have ripped over the years. Wish me luck
Solved my own problem. Downloaded CopyTrans Manager and it WORKED!!! Now, To get Apple to QUIT asking me to sign up for their music subscription service.
The top of my itunes screen on my PC says "updating files" "Waiting for items to copy" I have tried just dragging and dropping from Explorer also for the MP3s. No luck
Best of luck! 🙂
