What social media group?

App developers can choose that general grouping for their app, when they configure their app for availability within the app store. The social apps and the groupings in general are a fairly arbitrary grouping for apps. Other app groups shown within screen time include games (which uses a rocket icon), entertainment (striped bucket of popcorn icon), creativity (palette and brush icon), etc.

Do you have to apply the heart to it?

That’s the icon that Apple uses for the social media app group within screen time. It’s... decoration.
