I have promising news: I was able to record HB using Polar H7. The setup is following:
The outcome I have HB in iPhone Fitness App with Polar Beat logo some weird (some jumpy readings, like watch added on top of Polar, attached ) and in Strava with good quality as well (attached).

No. Apple cannot imagine that someone is using their watch as a fitness device. For that the internal sensor is not doing the trick and should be deactivated for good in order to save battery, no matter whether a chest belt is connected or not.
Dear Apple, please think about it.
I turned off the heart rate but then my iwatch 5 no longer communicated with my Powerlab chest monitor, only with my iphone. No data received on iwatch. If I turn the iwatch back on, I get data but probably not the Powerlab chest monitor data.
Thank you igal112 for sharing. I try to make it clear one more time to Apple, how ironic the situation is.
1) The Apple Workout App can receive data from external HR monitors (e.g. Polar) as long as the internal HR monitor (which is useless for sports) is generally activated.
2) Apple allows to deactivate the internal HR monitor while an external monitor is used without consequences.
3) But the Apple Workout App does not process data from external HR monitors anymore as soon as the internal monitor is deactivated for the time the external monitor is not connected.
So if I want to have my useless Apple HR monitor deactivated for good and only have an external monitor connected every now and then, I have every time to activate the internal monitor before connecting the external, for that the internal monitor would anyway be deactivated right away as soon as an external monitor is connected.
Apple does not want me to replace the internal monitor. Either I use the internal monitor and then I'm allowed to also use an external one or I have to forgo the HR function. Not very user friendly.
Dear Apple, do you see the irony in your software setting? Other fitness watch manufacturers may not produce as beautiful watches, though logically they are miles ahead. You can do better.
I got yesterday watch support expert on the call and he explained to me the following:
If you want to use an external HR (heart Rate) device,
If you switch off the Heart Rate in Privacy on the watch app on the phone, you won't get the heart rate in the fitness app on the watch from the Polar chest band any longer.
Since I activated the power saving mode during workouts, the internal sensor is deactivated while I do sports.
If you turn off the heart rate under privacy, you can't read the heart rate from the chest belt anymore either.
I don't understand why Apple connects the internal watch heart rate monitor with the chest belt, either both off or both on.
Watch app on phone,tap on privacy and turn off heart rate.
Exactly! Apple needs to fix this problem.
