This is a user-to-user forum. Apple doesn't read here for suggestions or feedback. You can, however, let them know your thoughts here:

Well, virtually all medical record systems identify you by your name and birth date, so it’s not a particularly useful medical ID for emergencies if it doesn’t have that information; without it an EMT or ER would not be able to look up your medical history. But you can always just not include your birth date in your medical ID. Open the Health app, tap on your icon in the upper right, tap Medical ID, Edit, and delete the birth date. You can put your age in the notes, but that’s not particularly useful in an emergency.

Your explanation and solution are not valid.

Birthdate is used in the Health app to determine maximum heart rate for physical exercise and possibly for other purposes. Removing it would therefore not be a good idea.

On the other hand, birthdate is used in the US for medical records, but not any other country where I've lived or visited. Even in the US, the birthdate is absolutely unnecessary for emergency care, especially if prescribed medications and pharmacological allergy information have been entered into the app and are subsequently displayed on the lock screen.

To reiterate... the birthdate being displayed on the lock screen for anyone (whether Emergency responders or phone thieves) is a serious breach of personal-data and violates Apple's data protection policies.
